This operation has been shown to be the best surgical procedure for the treatment of severe obesity and is the most common bariatric surgery performed at Holy Name Hospital.

First, a small stomach pouch is created to restrict food intake. The capacity of the stomach is reduced to only 30cc (equivalent to two tablespoons) by stapling across the stomach with a special metal device. Thus, approximately 90 percent of the stomach is below the staple line and is therefore isolated from the passage of food through the stomach.

Next, a Y-shaped section of the small intestine (typically 50-100 cm) is attached to the pouch to allow food to bypass the lower stomach, the duodenum (the first segment of the small intestine), and the first portion of the jejunum (the second segment of the small intestine).

The severely reduced size of the stomach in combination with the narrow diameter (1 cm) of the connection between the pouch and the small intestine are responsible for significant limitations placed on the patient's ability to eat large meals after the operation. These factors then allow patients to lose over 75 percent of their excess body weight over the first 18-24 months after surgery.
